What is Form PS-1583?
Form PS-1583, officially titled the Application for Delivery of Mail Through an Agent, is a required United States Postal Service (USPS) document.
This form authorizes a Commercial Mail Receiving Agency (CMRA)—commonly referred to as a virtual mailbox provider—to receive and handle mail on behalf of an individual or business.
Completing Form PS-1583
For the form to be legally valid, all fields must be completed accurately and in full, as even minor errors may lead to rejection.
Since notarization is required, the notary must ensure the form is completed exactly as instructed.
Each section below includes clear guidance on what information must be entered.
Overview of Form PS-1583
1. Private Mailbox (PMB) Information
This section is not completed during the notary session, as these fields are filled in directly by the virtual mailbox provider.
- Section 1a – Date PMB Opened: The date the mailbox was opened should be indicated here. This is not the date of notarization.
- Section 1b – Date PMB Closed: This field may remain blank. In many cases, the virtual mailbox provider enters N/A in this section.
2. Commercial Mail Receiving Agency (CMRA) Place of Business Information
The Applicant must provide the address and mailbox number of the virtual mailbox they intend to use. This should be the address of the mailbox location for which the Applicant is registered.
3. Type of Service Requested
In this section, the Applicant must indicate the purpose of the mailbox, specifying whether it is intended for personal use or business use.
Note :
- If the business option is selected, ensure that the business section (Section #7) is also completed by the Applicant.
- Confirm that the Applicant marks the correct option themselves and does not leave the field blank.
4. Name of Application & Address
Boxes 4a, 4b, 4c, 4d, 4e
In these fields, must be listed a Applicant personal information.
Boxes 4f, 4g, 4h, 4i, 4j
In these sections, must be listed a Applicant address information.
Typically, this section is filled out by the virtual mailbox provider, but the signer has the option to complete it themselves during the initial preparation for the notary session or at the session itself.
Note! The address in Sections 4 and 9b, 9c, 9d, 9e, and 9f must be the same. This address must also match the documents uploaded by the signer.
Box 4k
In this field, ‘Yes’ should be checked if there is an existing protection or restraining order (a copy of the court order is required), or ‘No’ if not.
5. Authorized Individual
This section should list the authorized individual’s personal and contact information, filled in by the Applicant or by the virtual mailbox provider, including name, contact details, and address.
Note! An authorized individual does not need to be present during the session; the Applicant must enter all personal data of the individual and upload the required IDs listed in sections 10 and 11.
6. If Transferring PMB Mail to Another Address
In this section, the Applicant provides the address and contact information where mail from their Private Mailbox (PMB) will be transferred.
7. Business/Organization Information
This section must be completed if Section #3 indicates that the mailbox is intended for business or organizational use.
If the mailbox is intended for personal use, all the fields in this section must be listed as “N/A”.
The Applicant must provide the business or organization name, type, address, contact number, and place of registration.
Note! For new or unregistered businesses, there cannot listed “N/A” for the place of registration. If there is no business address, the applicant’s home address from Section 4 must be entered in this field.
This address can match the address in boxes 4, but may NOT match the address in Section 2.
8. Photo ID Information for Applicant
In this section, the Applicant provides information from one ID, which must contain a photograph of the addressee.
This is the first of two IDs; the second ID will be provided in Section #9.

10. Photo ID Information for Authorized Individual (if applicable)
In this section, the Authorized Individual provides information from the one ID, which must contain a photograph of the person.

In this section, the Applicant needs to provide the authorized individual's information from the second ID, which must show their home address. This ID serves as proof of residence.
Note! In the document listed in Section 11g, the address information must match the address entered in Section 6e.

12. Exceptions for Additional Recipients of Mail
This section may include additional individuals authorized to receive mail under their own name.
- For Business/Organization Use:
List all members who will be receiving mail at this PMB
- For Residential/Individual Use:
The minor child's name can be added here if the form is a Residential form.
13. Signature of Applicant
The applicant must sign this section during the notary session.
Enter the date of signing in the designated field.
14. Signature of Witness
The Notary must sign this section during the notary session.

Important notes: Almost all fields may be automatically pre-filled by the virtual mailbox provider. All blank fields that do not require any information must be filled with N/A. This ensures that every section of the form is complete and no fields are left empty.

Once the application is 100% complete, accurate, and signed by the applicant, attach your ACKNOWLEDGMENT certificate.
As shown in the example of the filled-out document, type additional text:
- Under the applicant’s information: "SEE ATTACHED ACKNOWLEDGMENT"
- In a blank field: "This notarial act was an online notarization."
Note for Completing the Last Page AnytimeMailbox Only!:
For this business, the last page has a modified applicant section. It includes the following wording:
“The applicant, who proved to me on the basis of satisfactory evidence to be the person whose name is subscribed to this application, appeared before me, and did personally sign the application.”
Immediately after this sentence, and only for AnytimeMailbox cases, you must add the following text with the applicant’s ID information:
“The foregoing instrument was acknowledged before me, by means of Online Notarization, by [Signer’s Name], produced a [type of ID document, e.g., Florida Driver's License] as identification.”
