Form PS-1583 - most of you have seen this form and have notarized it once or twice.
So what is it anyhow?! Form PS-1583 is a United States Postal Service form that authorizes or designates an Agent to accept mail on behalf of another or entity. This is called an Application for Delivery of Mail Through Agent. In order to be legally valid the applicant must execute this form in duplicate in the presence of the agent, his or her authorized employee, or a notary public... therefore, notaries are vital to this form!
To successfully complete this form the entire document must be completed and done so correctly.
- Date
Always check this box for the date... enter today's date or correct the date to today's date;
To correct the date, use "XXXXXXXX" to strike through incorrect date, then enter the correct date above;
Finally, have the signer(s) initial the correction;
NOTE: today's date means the date that you and the signer(s) meet to notarize the document. - Name in Which Applicant's Mail Will Be Received to Agent
Here the applicant will enter their full legal name(s), if the virtual mailbox is for individual(s) or if the virtual mailbox is for a business, company or entity, they must enter the full legal name;

These boxes are for the applicant to enter the physical address of the business, company or entity;
This address will be used by the virtual mailbox provider to forward any mail the signatory requests to forward;
This address can be the same as boxes 7a, 7b, 7c, 7d but, may NOT be the same as 3a, 3b, 3c, 3d.
NOTE I: advise the signatory(s), this information will NOT be shared by the virtual mailbox provider with anyone but, the USPS.
NOTE II: if the virtual mailbox is for personal use, enter "N/A" in these boxes as well. - Type of Business
At a very high-level have the signatory enter what their business does or their industry;
i.e. Real Estate Sales, Retail, Consulting, Accounting, Mental Health Services, Legal Services, etc.
NOTE: if the virtual mailbox is for personal use, enter "N/A" in this box. - If applicate is a firm, name each member whose mail is to be delivered.
If boxes 9, 10a, 10b, 10c, 10d, 10e & 11 contain information then this box MUST contain the names of each person that will receive mail in the virtual mailbox;
All names listed MUST have verifiable identification information entered in boxes 8a & 8b;
A guardian may list the name(s) of all minors receiving mail in the virtual mailbox;
NOTE: if the virtual mailbox is for personal use, enter "N/A" in this box. - If a CORPORATION, Give Names and Addresses of Its Officers
This box is reserved for the name of all Corporate Officers for the business, company or entity;
Identification is NOT required for officers;
NOTE I: This is specific to corporations only; some examples of corporations: LLC, llc, Inc., Corp., S-Corp, C-Corp, Non-Profit, PLLC. etc.

If a business, company or entity name has been entered in boxes 2 & 9 the applicant MUST enter the County, State and date of formation;
This information can be found on the applicant's articles of incorporation;

Instead, enter the words, "See attached Certificate of Acknowledgment"
Then attach an ACKNOWLEGMENT certificate and complete it entirely, once the applicant completes box 16. - Signature of Applicant (if firm or corporation, application must be signed by officer. Show title.)
This box is designated for the applicant's signature, assign the signature box and advise the signatory to sign by drawing or uploading NOT by using the "type" signature feature;
If the application is for personal use, enter the signatory's name next to their signature;
If the application is for business use, enter the signatory's title next to their signature.
Once, the application is 100% complete, accurate and signed by the applicant, attach your ACKNOWLEGMENT certificate and complete it entirely. When in doubt, fill it out!
